Care Manager

A Care Manager is someone who will contact you, usually through your Managed Care Organization (MCO), to discuss your needs and how you might get long-term services and supports to help you remain independent and living in your own place. The care manager develops a care plan with you and visits you on a regular basis depending on where you live.

Your care manager can provide the following services:

  • Evaluate your service and care coordination needs on a regular basis and as your care needs change
  • Help you develop a plan for the care that you need
  • Help you select and arrange your services
  • Work with you and your doctors to ensure that all needed medical and dental visits and screenings take place
  • Help with service problems or concerns
  • Give you information about community resources that might be helpful to you
